Pathfinder Care is an Intensive Outpatient Program providing substance use and mental health treatment services in Saint Paul. Our 12 hours of weekly programming is offered Monday, Tuesday, Thursday, and Friday from 9:00 a.m. to 12:00 p.m.

Pathfinder Care also offers a Community Assessment Program, providing substance use assessments to help individuals access the appropriate level of care and support.

Through our integrated connection technology, Pathfinder Care supports recovery through enhanced communication, engagement, recovery signals, care coordination, and ongoing connection between clients and care teams throughout the recovery journey.
